PIAA Physical Forms
All student-athletes are required to have sections 1 through 6 of the PIAA physical forms uploaded and approved in their Magnus Health account in order to participate in athletics. Section 7 is an additional section that is required between each sports season that a student-athlete participates in. DC partners with Magnus Health to manage all health forms and medical records. When are PIAA Physicals due?
PIAA physicals should be uploaded and approved in Magnus by the start of each sports season. Due dates vary by season. No student will be permitted to participate without an approved physical on file.
Where does my PIAA Physical go once it is completed by my doctor?
PIAA Physical Forms should be uploaded to your Magnus Health Portal under “PIAA Physical Forms 24-25” for approval.
How do I access my Magnus Health Portal?
Magnus can be accessed through your Blackbaud account. Once you log in click “resources” and you will see a tile with a link to Magnus Health.
Where can I go to get a physical completed?
You can visit your family doctor or visit a number of urgent cares to have this done.
Where can I find the PIAA forms?
PIAA forms are available in Magnus Health or you can access them here.
What is Section 7 - Parent Recertification Form?
Section 7 is an additional page of the PIAA forms that is required in between each additional sports season to update emergency contact information and health history. For example, a soccer player who plays basketball and lacrosse will have to submit Section 7 between the fall and winter seasons and between the winter and spring.
