PIAA Physical Evaluation

All students in grades 6-12 must submit a PIAA Pre-Participation Physical Evaluation form. DC partners with Magnus Health to manage all health forms and medical records. Parents can access their Magnus Health Account by logging into their Facts Family Online Portal and selecting the Medical tab.
