Submit your child's 2024-2025 PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) Form!

All student-athletes must have an approved physical on file before they will be permitted to play. The PIAA forms can be found HERE. Sections 1-6 are required for all student-athletes and are valid for the entire school year. Parents should upload the completed forms into their Magnus Health account. You can access Magnus through your Blackbaud account: choose resources, and then click on the Magnus tile. PIAA physicals for the fall season are due by August 1!
